ارتباط پیش دیابت و دیابت با عوامل خطر قلبی- عروقی: یک مطالعه مورد- شاهدی

چکیده

مقدمه: بیماری دیابت قندی نوع دو و پیش دیابت به دنبال مواردی چون چاقی، اختلالات چربی خون و پرفشاری خون بروز می‌کند. هدف از انجام این مطالعه، شناسایی عوامل خطر این دو نوع دیابت بود.
روش کار: در این مطالعه مورد- شاهدی، 29 نفر در گروه دیابتی‌ها، 140 نفر در گروه پیش دیابتی و 169 نفر همسان شده ازنظر سن و جنسیت در گروه شاهد قرار گرفتند. شاخص‌های آنتروپومتریک و فشارخون گروه‌ها به روش استاندارد انجام شد. برای اندازه‌گیری تری گلیسرید، قند خون ناشتا، کلسترول تام، لیپوپروتئین با چگالی پایین (LDL) و لیپوپروتئین با چگالی بالا (HDL) از افراد هر سه گروه نمونه‌های خونی گرفته شد. رابطه بین متغیرها با استفاده از آزمون‌های مجذور کای، تی مستقل، تحلیل واریانس یک‌طرفه و رگرسیون لجیستیک سنجیده شد. ارزش پی (p < /span>) کمتر از 05/0 به‌عنوان سطح معنادار آماری در نظر گرفته شد.
یافته‌ها: با تحلیل رگرسیون لجیستیک مشخص شد که هیپرتری گلیسریدمی (46/5-54/1 :90/2 =OR) و LDL بالا (40/14-35/1 :42/4 =OR) از عوامل خطر اصلی ابتلا به‌پیش دیابت و شاخص توده بدنی (28/1-03/1 :15/1 =OR)، هیپرتری گلیسریدمی (67/8-35/1 :43/3 =OR) و فشارخون سیستولی (06/1-01/1 :03/1 =OR) از عوامل خطر ابتلا به دیابت قندی در مقایسه با افراد گروه شاهد است. از طرفی فشارخون دیاستولی (08/1-01/1 :04/1 =OR) عامل خطر مهم در بیماران دیابتی در مقایسه با افراد پیش دیابتی بود.
بحث و نتیجه‌گیری: طبق نتایج این مطالعه، شاخص توده بدنی بالا، هیپرتری گلیسریدمی، LDL بالا و میزان فشارخون می‌تواند در پیشگیری و کاهش پیش دیابت و دیابت مؤثر باشد.

Relation of Pre-diabetes and Diabetes Mellitus and Cardiovascular Risk Factors: A Case-Control Study

Introduction:
Diabetes mellitus type 2 and pre-diabetes are triggered by a variety of factors such as obesity, lipid disorders and hypertension. The aim of study was to determine diabetic and pre-diabetic risk factors.
Material and Methods:
In this case-control study, a total of 29 individuals were enrolled in the diabetic group, 140 individuals in pre-diabetic group and 169 age- and sex-matched individuals (normal fasting blood sugar) in the control group. Anthropometric indexes and blood pressure were measured by standard methods. Blood samples were taken from the three groups for measuring triglyceride, fasting blood sugar, cholesterol, low density lipoprotein and high density lipoprotein. Relationships between variables were tested using chi-square test, independent t-test, one-way ANOVA, and binary logistic regression models. P< 0.05 was considered as statistically significant.
Results:
With logistic regression, the important risk factors in pre-diabetics were hypertriglyceridemia (OR= 2.90: 1.54-5.46) and high LDL cholesterol (OR= 4.42: 1.35-14.40) compared to subjects with normal blood sugar. In diabetics, the important factors were body mass index (OR= 1.15: 1.03-1.28), hypertriglyceridemia (OR= 3.43: 1.35-8.67) and systolic blood pressure (SBP) (OR= 1.03: 1.01-1.06) compared to subjects with normal blood sugar. On the other hand, diastolic blood pressure (OR= 1.04: 1.01-1.08) was the important factor in diabetic patients compared to pre-diabetes.
Conclusion:
According to the results of this study, attention to high body mass index, hypertriglyceridemia, high LDL and blood pressure can be effective on prevention and reduction of pre-diabetes and diabetes mellitus.
