ارتباط مصونیت القایی به سیاه‌سرفه با سطح سرمی شاخص‌های آهن و آمینوترانسفراز‌‌‌ها در بیماران تالاسمی وابسته به دریافت خون

چکیده
مقدمه:
سیاه‌سرفه یک بیماری تنفسی قابل پیشگیری با واکسن است که در افراد مبتلا به بیماری‌های زمینه‌ای می‌تواند منجر به عوارض، مرگ و میر و هزینه‌های قابل توجهی شود. پایش مصونیت حاصله از واکسیناسیون در بیماران تالاسمی بعنوان یکی از گروه های پرخطر نسبت به بیماریهای عفونی، از اهمیت بالایی برخوردار است. این تحقیق به منظور بررسی ارتباط وضعیت مصونیت سیاه‌سرفه با شاخص‌های آهن و آمینوترانسفراز‌‌‌ها در بیماران تالاسمی واکسینه شده علیه سیاه‌سرفه انجام شد.
روش کار: این مطالعه مقطعی-توصیفی در سال 1403 بر روی 85 بیمار تالاسمی وابسته به دریافت خون شهرستان جهرم (سرشماری) انجام شد. اطلاعات دموگرافیک، سطح سرمی فریتین و آمینوترانسفرازها از پرونده بیماران استخراج شد. میزان سرمی آنتی‌بادی علیه بوردتلا پرتوسیس به روش الایزا تعیین شد. داده ها پس از گردآوری با نرم‌افزار SPSS-22 و با استفاده از آمار توصیفی و تحلیلی مورد تجزیه و تحلیل قرار گرفت.
یافته‌ها:‌ از 85 بیمار مورد مطالعه، تعداد 6/47درصد زن و 4/52درصد مرد بودند که 2/81درصد مصون و 8/18درصد غیرمصون بودند. تفاوت معنی‌داری از نظر سن، جنس و سابقه برداشتن طحال با وضعیت مصونیت به سیاه‌سرفه وجود نداشت(P>0.05) . میانگین سرمی شاخص های آهن در بیماران مصون و غیرمصون تفاوت آماری معنی‌داری داشتند (p<0.05)، ولی میانگین آمینوترانسفرازها در بیماران مصون و غیرمصون تفاوت معنی داری نداشت (p>0.05).
نتیجه‌گیری:‌ در بیماران مورد مطالعه، مصونیت نسبت به سیاه‌سرفه با گرانباری آهن ارتباط معکوس معنی‌دار دارد، ولی با میزان سرمی ترانسفرازها ارتباط اماری معنی‌داری ندارد. تحقیقات بیشتر به منظور ارائه راهکارهای بهتر برای اثربخشی واکسیناسیون در این بیماران پیشنهاد می شود.

Background: Pertussis is a vaccine-preventable respiratory disease that can lead to significant complications, mortality, and costs in people with underlying diseases. Monitoring the immunity resulting from vaccination in thalassemia patients, as one of the high-risk groups for infectious diseases, is important. This study was conducted to investigate the relationship between the status of pertussis induced immunity and serum level of iron indexes and aminotransferases in pertussis vaccinated thalassemia patients.
Methods: This cross-sectional descriptive study was conducted on 85 thalassemia patients (by census) in Jahrom-2025. Demographic information, serum ferritin and aminotransferase levels were extracted from the patients' records. Serum levels of antibodies against Bordetella pertussis were determined by ELISA. After collecting the data, they were analyzed using SPSS-22 software and descriptive and analytical statistics.
Results: Of the 85 patients studied, 47.6% were female and 52.4% were male, 81.2% were immune and 18.8% were non-immune. There was no significant difference in terms of age, gender, and history of splenectomy with pertussis immunity status (P>0.05). Mean serum iron indices in immune and non-immune patients were statistically significantly different (p<0.05), but there was no significant difference in mean aminotransferases in immune and non-immune patients (p>0.05).
Conclusion: In the patients studied, immunity to pertussis was significantly inversely associated with iron overload, but not with serum levels of transferase. Further researches are recommended to provide better strategies for the effectiveness of vaccination in these patients.
