نقش ناگویی خلقی و تجارب نامطلوب دوران کودکی در پیش‌بینی ولع مصرف در افراد وابسته به مواد با نقش میانجی عملکرد نامتناسب خانواده: رویکرد مدل معادلات ساختاری

چکیده
مقدمه: ولع مصرف یکی از ابعاد مهم در درک و تبیین وابستگی به مواد است که عوامل متعددی در شکل‌گیری آن نقش دارند.
پژوهش حاضر با هدف بررسی نقش ناگویی خلقی و تجارب ناخوشایند دوران کودکی در پیش‌بینی ولع مصرف در افراد وابسته به مواد، با درنظر گرفتن نقش میانجی عملکرد نامتناسب خانواده و با بهره‌گیری از رویکرد مدل‌سازی معادلات ساختاری انجام شد.
روش‌کار: این مطالعه از نوع مقطعی و مبتنی بر تحلیل ساختاری بود. ۲۸۰ نفر با روش نمونه‌گیری خوشه‌ای تصادفی چندمرحله‌ای انتخاب شده و به پرسش‌نامه‌های ولع مصرف Somoza، عملکرد نامتناسب خانواده Epstein، ناگویی خلقی Bagby و تجارب نامطلوب دوران کودکی Bernstein پاسخ دادند. داده‌ها با استفاده از نرم‌افزارهای SPSS نسخه ۲6 و SmartPLS نسخه ۳ و در قالب مدل معادلات ساختاری تحلیل شدند.
یافته‌ها: یافته‌ها نشان دادند که ناگویی خلقی اثر مستقیم و معناداری بر ولع مصرف و بر عملکرد نامتناسب خانواده دارد. همچنین تجارب نامطلوب دوران کودکی اثر معناداری بر ولع مصرف و عملکرد نامتناسب خانواده گذاشت. علاوه بر این، نتایج نشان داد که ناگویی خلقی از طریق عملکرد نامتناسب خانواده به‌طور غیرمستقیم بر ولع مصرف اثرگذار است به همین ترتیب، تجارب نامطلوب دوران کودکی نیز از طریق عملکرد نامتناسب خانواده اثر غیرمستقیمی بر ولع مصرف داشت
نتیجه‌گیری: توجه به تجارب هیجانی و خانوادگی افراد، به‌ویژه در دوران کودکی، از اهمیت بالایی در درمان و پیشگیری از ولع مصرف و بازگشت به مصرف برخوردار است. تقویت مداخلات روان‌درمانی فردی و خانوادگی، آموزش مهارت‌های تنظیم هیجان و بهبود عملکرد خانواده، به‌ویژه در مراحل اولیه درمان، توصیه می‌شود.

The Role of Alexithymia and Adverse Childhood Experiences in Predicting Substance Craving among Individuals with Substance Dependence: The Mediating Role of Dysfunctional Family Functioning through Structural Equation Modeling Approach

Introduction: Craving is a key dimension in understanding and explaining substance dependence, influenced by multiple psychological and environmental factors. This study aimed to investigate the role of alexithymia and adverse childhood experiences in predicting substance craving among individuals with substance dependence, considering the mediating role of dysfunctional family functioning through a structural equation modeling approach.
Materials and Methods: This study was cross-sectional in design and based on structural analysis. A total of 280 participants were selected using multistage cluster random sampling and completed the Somoza Drug Craving Questionnaire, the Epstein Dysfunctional Family Functioning Scale, the Bagby Toronto Alexithymia Scale, and the Bernstein Childhood Trauma Questionnaire. Data were analyzed using SPSS version 26 and SmartPLS version 3 within the framework of structural equation modeling.
Results: Results showed that alexithymia had a direct and significant effect on drug craving (T=4.51, β=0.64) and on dysfunctional family functioning (T=4.89, β=0.51). In addition, adverse childhood experiences had a significant effect on drug craving (T=3.01, β=0.55) and on dysfunctional family functioning (T=12.04, β=0.72). Furthermore, the findings indicated that alexithymia indirectly influenced drug craving through dysfunctional family functioning (T=2.10, β=0.27). Similarly, adverse childhood experiences also had an indirect effect on drug craving via dysfunctional family functioning (T=2.98, β=0.38).
Conclusion: Based on the results, emotional and familial experiences—especially during childhood—play a significant role in the development and maintenance of substance craving. Enhancing individual and family-based psychotherapeutic interventions, emotional regulation training, and improving family functioning, particularly in the early stages of treatment, are recommended.
