بررسی ارتباط تغییرات مودیک وتورم مفصل بر پیامد های بالینی، عملکردی و پایداری ستون فقرات در بیماران عمل شده دژنراتیو لومبار

چکیده
مقدمه:  هدف این مطالعه، بررسی ارتباط بین تغییرات مودیک و تورم مفصل و همچنین رابطه آن با پایداری ستون فقرات لومبار در بیمارانی بود که به دلیل اختلالات دژنراتیو تحت عمل جراحی قرار گرفتند.
روش کار:  این مطالعه مشاهده ای روی داده های جمع آوری شده به صورت گذشته نگر از پرونده ۱۱۶ نفر از مراجعه کنندگان به بیمارستان شهید چمران شیراز در بین سال های 1396 تا 1399 انجام شد.  پرونده بیماران در ابتدا از نظر متغیرهای زمینه ای میزان تورم مفصل (Facet Effusion)، تغییرات مودیک و نوع عمل بررسی شد. بیماران پس ازشش ماه تحت بررسی بالینی و تصویربرداری قرار گرفتند و پایداری ستون فقرات بررسی شد.داده ها با استفاده از آمار توصیفی و استنباطی تجزیه و تحلیل شدند.
یافته ها: شیوع ناپایداری 6 ماهه در این مطالعه بسیار کم بود (1 بیمار، 0.86 درصد) و تنها بیماری که دچار ناپایداری ستون فقرات شده بود، جراحی لامینکتومی در سطح 2 انجام داده بود؛ دیسککتومی نشده بود و تغییرات مودیک در زمان جراحی نداشت. مقدار Facet Effusion و Dynamic angulation این بیمار به ترتیب 1.50 میلی متر و 5 میلی متر بود و Dynamic Translation  و vacuum sign نداشت.  مقدار میانگین Dynamic angulation ،Facet Effusion و Dynamic Translation در کل بیماران به ترتیب برابر 0.58 ± 1.56، 2.67±5 و 0.51±3 میلی متر بود.
نتیجه گیری: یافته های این مطالعه با توجه به شیوع بسیار پایین ناپایداری در پیگیری شش ماهه نمی تواند برای نتیجه گیری در مورد عوامل موثر در برور ناپایداری کمر مورد استفاده قرار گیرد و نیاز به مطالعات گسترده تری است.  با این حال، دلایل شیوع بسیار پایین ناپایداری کمری در این مطالعه به عوامل متنوع دیگری و از جمله روش جراحی ارتباط داشته که می تواند با سایر مطالعات با شیوع ناپایداری بیشتر مورد مقایسه قرار گیرد.

Evaluation of the relationship of modic change and effusion of facet effects on clinical, functional and stability of spine in operated patients with lumbar degenerative diseases

Introduction: The aim of this study was to investigate the relationship between Modic changes and facet effusion and
its relationship with lumbar spine stability in patients who underwent surgery for degenerative disorders.
Materials and Methods: This observational study was performed on retrospectively collected data from the records of 116
patients referred to Shahid Chamran Hospital in Shiraz between 2016 and 2019. Patients' records were initially queried for demographic data, facet effusion extent, modic changes and type of operation. Patients underwent clinical examination and imaging after 6 months and spinal stability was assessed. Data analysis was performed using descriptive and inferential statistical tests.
Results: The prevalence of 6-month instability in this study was very low (1 patient, 0.86%) and the only patient with spinal instability underwent level 2 laminectomy with no discectomy nor Modic changes at the time of surgery. His facet effusion was 1.50 mm; He had a dynamic angulation of 5 mm and no dynamic translation or vacuum sign. Mean facet effusion, dynamic angulation and dynamic translation in all patients were 1.56±0.58, 5±2.67 and 3 ± 0.51 mm, respectively.
Conclusion: The findings of this study due to the very low prevalence of instability in 6-month follow-up cannot be used to draw a conclusion about the factors affecting lumbar instability and advanced studies are needed. However, the reasons for the very low prevalence of lumbar instability in this study may be related to surgical technique and a wide variety of other factors that can be compared with other studies with a higher prevalence of instability.
