اثر افزودن فنتانیل به بوپیواکایین روی لرز، تهوع و استفراغ بیماران تحت عمل جراحی شکستگی فمور با روش بی حسی نخاعی: یک مطالعه کارآزمایی بالینی تصادفی شده دو سویه کور

چکیده
مقدمه: از شایع ترین عوارض بعد از بی حسی نخاعی، لرز حین عمل است. دومین شکایت شایع گزارش شده به دنبال بیهوشی داخل نخاعی حالت تهوع و استفراغ  است که این  عوارض جانبی هنوز هم در طی مراحل جراحی وجود دارد و باعث ناراحتی بیماران می شود. هدف از مطالعه حاضر تعیین اثر افزودن  فنتانیل به بوپیواکایین روی لرز و تهوع و استفراغ بیماران تحت عمل جراحی فمور با روش
بی حسی نخاعی بود.
روش کار: این مطالعه کارآزمایی بالینی تصادفی شده دو سویه کور روی 40 بیمار 18 تا 50 سال با کلاس بیهوشی 1و2 که منتخب عمل جراحی ارتوپدی فمور بودند، انجام شد. بیماران به صورت تصادفی به دو گروه مساوی تقسیم شدند. گروه مداخله، 12.5 میلی گرم بوپیواکایین (2.5 سی سی) به همراه 10 میکروگرم فنتانیل و گروه شاهد، 12.5 میکروگرم بوپیواکایین و نیم میلی لیتر نرمال سالین دریافت کردند.  در زمان های 2،1، 6، 12 و 24 ساعت پس از عمل، عوارض دارویی شامل خشکی دهان ، تهوع ، استفراغ  و لرز در بیماران مورد ارزیابی قرار گرفت. داده ها به روش آمار توصیفی و استنباطی با استفاده از آزمون های آماری تجزیه و تحلیل شدند.
یافته ها: نتایج نشان داد که بین دو گروه دریافت کننده بوپیواکائین- فنتانیل و بوپیواکائین از لحاظ فراوانی تهوع در ریکاوری و زمان‌های بعد از عمل، تفاوت آماری معناداری وجود ندارد (p>0.05). همچنین بین دوگروه از نظر عارضه جانبی لرز در زمان های 18،12،6و24ساعت بعد از عمل، تفاوت معناداری وجود نداشت (p>0.05).
نتیجه گیری: در مطالعه حاضر اگرچه در دقایقی از مطالعه در گروه فنتانیل عوارضی مانند لرز و تهوع و استفراغ کمتر گزارش شد؛ اما این کاهش از لحاظ آماری معنادار نبود. پیشنهاد می شود در مطالعات بعدی از دوزهای دیگری از فنتانیل برای امکان مقایسه با سایر
مطالعه ها استفاده شود.

چکیده English

Introduction: One of the most common complications after intrathecal anesthesia is chills during surgery. The second
most common complaint followed by intrathecal anesthesia is nausea and vomiting. The aim of this study was to evaluate the effect of adding fentanyl to bupivacaine on chills and vomiting nausea in patients undergoing femoral surgery by spinal anesthesia.
Materials and Methods: This randomized double-blind clinical trial study was performed on 40 patients aged 18 to 50 years with anesthesia classes 1 and 2 who were candidates for femoral orthopedic surgery. Patients were randomly
divided into two equal groups. The intervention group received 12.5 mg bupivacaine (2.5 cc) and 10 mg fentanyl (control group) and the control group received 12.5 μg bupivacaine (0.5 ml normal).
Patients with dry mouth, nausea, vomiting and shivering were evaluated at 1, 2, 6, 12 and 24 hours after surgery. Data analysis was performed using SPSS software version 21 and descriptive and inferential statistical tests.
Results: Forty patients undergoing spinal orthopedic surgery with spinal anesthesia participated in two equal groups (bupivacaine-fentanyl (n = 20) and bupivacaine (n = 20). There was no significant difference between fentanyl and bupivacaine in terms of recovery from nausea and postoperative times (p> 0.05) and differences between bupivacaine-fentanyl and bupivacaine in shivering at 6, 12, 18 and 24 hours postoperatively. There was no significant (p> 0.05).
Conclusion: In the present study, although fewer complications such as shivering and nausea and vomiting were
reported within minutes of the study in the fentanyl group, this decrease was not statistically significant.
