چکیده
مقدمه: لرزی که پس از بی هوشی بروز می کند باعث احساس ناراحتی در بیمار می شود. بسیاری از داروها در رابطه با جلوگیری یا کاهش لرز بررسی شده اند، اما هنوز دارویی ایده آل مورد تایید نهایی قرار نگرفته است. هدف از مطالعه حاضر بررسی تاثیر تجویز
دکس مدتومدین به همراه بوپیواکایین در روش بی حسی نخاعی روی پیشگیری از بروز لرز در عمل های جراحی ارتوپدی شکستگی تیبیا و فمور بود.
روش کار: این مطالعه کارآزمایی بالینی تصادفی روی 60 بیمار 18تا 50 سال با کلاس بی هوشی (ASA class) 1 و 2 که منتخب عمل جراحی ارتوپدی شکستگی تیبیا و فمور بودند، در سال 1397 انجام شد. بیماران به صورت تصادفی به دو گروه تقسیم شدند؛ گروه مداخله: 12.5 میلی گرم بوپیواکایین (2.5 سی سی) و 5میکروگرم دکس مدتومدین (0.5 میلی لیتر) و گروه شاهد: 12.5 میکروگرم بوپیواکایین و 0.5 میلی لیتر نرمال سالین دریافت کردند. مقادیر فشارخون سیستولیک، دیاستولیک و متوسط خون شریانی، ضربان نبض و درصد اشباع اکسیژن خون در زمان های قبل از بی حسی نخاعی، بعد از بی حسی و سپس هر از 15دقیقه تا اتمام عمل جراحی و در زمان ریکاوری ثبت شد. بیماران در زمانهای 2، 4، 8، 12 و 24 ساعت پس از عمل از نظر میزان لرز مورد ارزیابی قرار گرفتند. تجزیه و تحلیل داده ها با شاخص های آمار توصیفی میانگین، درصد و انحراف معیار و آزمون های آمار استنباطی مربع کای، تی تست، من ویتنیی یو با استفاده از نرم افزار SPSS نسخه 21 انجام شد.
یافته ها: میانگین سنی بیماران در گروه شاهد 14.7 ± 36.6 سال و در گروه مداخله 8.6 ± 32.7 سال بود. دو گرو ه مداخله وشاهد از لحاظ فراوانی لرز در زمانهای 2 (4.3% و 7.1%) و 4 (4.5% و 24.1%) ساعت بعد از عمل تفاوت معناداری نداشتند. هر چند افراد گروه مداخله در ساعت های 8، 12 و 24 بعد از عمل جراحی هیچ گونه لرزی را گزارش نکردند اما با گروه شاهد (7.1%، 3.6% و 3.6%) تفاوت معنادار نداشتند.
نتیجه گیری: نتایج مطالعه نشان می دهد که افزودن 5 میکروگرم دکس مدتومدین به بوپیواکایین در روش بی حسی نخاعی باعث کاهش میزان بروز لرز بعد از عمل می شود، ولی این اختلاف از نظر آماری معنادار نیست. بنابراین به نظر می رسد در این زمینه نیاز به مطالعات جامع تر و با حجم نمونه زیادتری است.

Effect of Dexmedetomidine in combination with intrathecal bupivacaine on the incidence of shivering after orthopedic surgery of the femur and tibia: a double-blind randomized clinical trial study

Introduction: Post-anesthesia shivering causes discomfort in the patient. Many drugs have tried to prevent or reduce
shivering, but the ideal drug has not been finalized yet. The purpose of this study was to determine the effect of Dexmedetomidine with bupivacaine in spinal anesthesia to prevent shivering in orthopedic fracture surgery of femur and tibia.
Materials and Methods: This randomized clinical trial study was performed on 60 patients 18-50 years old with ASA class 1 and 2 who were candidates for tibia and femur orthopedic fracture surgery in 2018. Patients were randomly divided into two groups; Intervention group: 12.5 mg bupivacaine (2.5 cc) and 5 μg Dexmedetomidine (0.5 ml) and control group: 12.5 mg bupivacaine and 0.5 ml normal. Systolic, diastolic, and mean arterial blood pressure and blood O2% saturation were recorded before spinal anesthesia, after anesthesia and then every 15 minutes until surgery and recovery time. At 2, 4, 8, 12 and 24 hours postoperatively shivering was evaluated. Data were analyzed using descriptive statistics indices (mean, percentage and standard deviation) and inferential statistical tests (chi-square, t-test, and Mann-Whitney u) using SPSS 21 software.
Results: The mean age of patients in the control group was 36.6 ±14.7 years and the intervention group was 32.7± 8.6 years. There was no significant difference in the frequency of shivering between the control and intervention groups at 2 (4.3% vs. 7.1%) and 4 (4.5% vs. 24.1%). Although, intervention subjects had no any shivering state at hours of 8, 12 and 24, but there was no significantly different compared to control group (7.1%, 3.6% and 3.6%, respectively).
Conclusion: The results of study showed that addition of 5 μg Dexmedetomidine to bupivacaine in spinal anesthesia
reduces the incidence of postoperative shivering, but this difference was not statistically significant. Thus, in this subject it seems bee need to studies with whole and large sample of participants.
