بررسی مقاومت آنتی بیوتیکی و تشکیل بیوفیلم در باکتری های ایزوله شده از دستگاه تنفسی فوقانی بزرگسالان بستری در بیمارستان شهید باهنر کرمان

چکیده

مقدمه: عفونت تنفسی فوقانی از شایع ترین عفونت های باکتریایی در انسان و دومین دلیل تجویز بالای آنتی بیوتیک ها است. روند صعودی مقاومت آنتی بیوتیکی یک چالش جهانی محسوب می شود. این پژوهش با هدف بررسی مقاومت آنتی بیوتیکی و تشکیل بیوفیلم در باکتری های ایزوله شده از دستگاه تنفسی فوقانی بیماران بزرگسال بستری در بیمارستان شهید باهنر کرمان انجام گرفت.
روش کار:  در این تحقیق 70 نمونه تنفسی کشت داده شده از آزمایشگاه بیمارستان شهید باهنر در کرمان در یک بازه زمانی سه ماهه دریافت شد . سپس بررسی و تشخیص باکتری ها با آزمون های بیوشیمیایی متداول و آزمون حساسیت آنتی بیوتیکی به روش انتشار دیسک انجام شد. با تعیین درصد چسبندگی سطحی ایزوله ها، تشکیل بیوفیلم روی سطوح شیشه ای و پروپیلن در شرایط تکان و ثابت بررسی شد.
یافته ها: تعداد 59 باکتری ایزوله شده به ترتیب شامل: استافیلوکوکوس آرئوس (37.28 درصد)، کلبسیلا پنومونیه (20.33 درصد)، سودوموناس آئروژینوزا (16.94 درصد)، اشرشیا کلی (6.77 درصد)، استرپتوکوکوس پایوژنز (5.08 درصد) و سراشیا مارسسنس، پروتئوس میرابیلیس، کورینه باکتریوم دیفتریه و گونه های سیتروباکتر (3.38 درصد) بودند. بیشترین موارد مقاومت آنتی بیوتیکی به تتراسایکلین و بیشترین موارد حساسیت به جنتامایسیسن مشاهده شد. توان تولید بیوفیلم بر سطح پروپیلن و در حالت متحرک  جذب بیشتری نشان داد.
نتیجه گیری: براساس نتایج به دست آمده از پژوهش حاضر، بین مقاومت آنتی بیوتیکی و توان تولید بیوفیلم در ایزوله ها، ارتباط مستقیمی مشاهده شد. می توان با ارزیابی الگوی حساسیت آنتی بیوتیک ها و توسعه روش های درمانی جدید، برنامه ای هدفمند و موثر تدوین کرد.

Introduction: Upper respiratory tract infection is one of the most common bacterial infections in humans and the
second reason for the high administration of antibiotics. The upward trend of antibiotic resistance in bacterial pathogens is a worldwide challenge. The aim of the present study was to evaluate antibiotic resistance and biofilm formation of isolated bacteria from the upper respiratory tract of adult patients admitted to Shahid Bahonar Hospital in Kerman.
Materials and Methods: In this study, 70 sub cultured specimens of respiratory were received from laboratory of Shahid Bahonar Hospital in Kerman during three months. Examination and identification was done by routine biochemical tests. The disc diffusion antibiotic susceptibility test was also performed. Next, the percentage of cell surface hydrophobicity was determined. The isolates with highest and lowest hydrophobicity were selected to study biofilm formation on glass and polypropylene surfaces on static and shake conditions.
Results: 59 bacterial strains were isolated, respectively as %37.28 Staphylococcus aureus., %20.33 Klebsiella pneumonia, %16.94 pseudomonas aeruginosa, %6.77 Escherichia coli, %5.08 Streptococcus pneumonia and %3.38 Citrobacter spp., Serratia marcescen, Proteus mirabilis and Corynebacterium diphtheria. Maximum resistance was related to amoxicillin and all the strains were sensitive to gentamicin. The results of biofilm formation on the selected surfaces indicated biofilm formation increasing on propylene surfaces with shake condition.
Conclusion: According to the results, there is a direct relationship between antibiotic resistance and biofilm formation potential of isolates. An effective program can be developed by evaluating antibiotics susceptibility patterns and new therapeutic methods.
