اپیدمیولوژی عفونت های بیمارستانی در بیماران بستری در بیمارستان های شهر جهرم در سال 1395

چکیده

مقدمه : افزایش بیمارستان ها، ظهور بیماری های نوپدید و بازپدید، افزایش روز افزون مقاومت های میکروبی و نیاز به خدمات متنوع پزشکی، بروز عفونتهای ناشی از مراقبت های درمانی را اجتناب ناپذیر می سازد.

 روش کار :  این مطالعه بصورت مقطعی روی پرونده ی بیمارانی که در سال 1395 با تشخیص عفونت بیمارستانی در بیمارستان های شهر جهرم ثبت شده اند انجام شد. چک لیستی بر اساس اهداف طرح طراحی شد. داده ها با استفاده از نرم افزارspss نسخه 16 با آمار توصیفی و آزمون کای اسکوئر در سطح معنی داری05/0= α مورد تجزیه و تحلیل قرار گرفت.

یافته ها : شیوع عفونت های بیمارستانی در این مطالعه 32 در ده هزار بیمار بود. بیشترین شیوع عفونت های بیمارستانی مربوط به بخشNICU(1/342 در ده هزار نفر)، اطفال(3/92 در ده هزار نفر)  و بخش داخلی (14/68 در ده هزار نفر) بود. عفونت های بیمارستانی شایع در بیمارستان های شهر جهرم به ترتیب: عفونت دستگاه ادراری(2/54 درصد)، عفونت محل جراحی(9/29 درصد)، پنومونی(9/11 درصد) و عفونت دستگاه گوارش(GIT)( 4 درصد) بود.

نتیجه گیری: با توجه به فراوانی بالای استفاده از اقدامات مداخله ای در بیماران دچار عفونت بیمارستانی می بایست این اقدامات در شرایط خاص و در حداقل موارد استفاده شود. همچنین، بایستی رعایت بهداشت کامل به کارکنان و کادر پزشکی بخش ها آموزش داده شود. تشخیص به موقع، مراقبت و گزارش تمامی موارد عفونت های بیمارستانی در سیستم نظام مراقبت موجود می تواند در کاهش موارد جدید و همچنین عوارض این عفونت ها موثر باشد.

چکیده [English]

Introduction: Establishment of new hospitals, incidence of emerging and re-emerging diseases, increasing microbial resistance and the need for diversified medical services have increased the incidence of Nosocomial infections.

Materials and methods: In this cross-sectional study, records of patients admitted to Jahrom hospitals in 2016 and diagnosed with Nosocomial infections were studied. An objective-based checklist was designed. Date were analyzed using SPSS 16 and by utilizing descriptive statistics and chi-square test at 0.05 significance level

Results: The prevalence of Nosocomial infections was 32 in 10000 patients. The highest incidence of Nosocomial infections was observed in the NICU (342.1 in 10000), pediatrics (92.3 in 10000) and the internal ward (68.14 in 10000), respectively. The most prevalent Nosocomial infections cases in Jahrom hospitals included: Urinary tract infections (54.2%), surgical site infections (29.9%), pneumonia (11.9%) and gastrointestinal tract infections (GTIs) (4%).

Conclusion: Considering the numerous intervention measures in patients with Nosocomial infections, such measures should be performed under specific conditions and only in vital cases. Medical and non-medical staff should be trained to maintain personal and public hygiene. In existing care system, timely diagnosis, providing thorough care and reporting all Nosocomial infections can reduce new cases and their complications.
