چکیده
زمینه و اهداف: به دلیل وجود گونه‌های مختلف انتروکوکوس، شناسایی دقیق انتروکوک‌ها در سطح گونه از اهمیت بالایی برخوردار می‌باشد، لذا هدف این مطالعه استفاده از روش‌های فنوتیپی و ژنوتیپی برای تشخیص گونه‌های انتروکوکوس ایزوله شده از بیماران مراجعه‌کننده به بیمارستان‌های شهر تهران و بررسی الگو مقاومت داروئی در آن‌ها بود.

روش بررسی: این مطالعه آزمایشگاهی بر روی 400 نمونه بالینی از بیمارستان‌های شهر تهراندر طی سال‌های 93-1394 صورت پذیرفت. برای شناسایی انتروکوک از کشت‌های اختصاصی استفاده شد. از آزمون اختصاصی بیوشیمیایی برای شناسایی گونه‌های انتروکوکوس فکالیس و انتروکوکوس فاسیوم استفاده گردید. از روش PCR به‌منظور شناسایی گونه‌های انتروکوک استفاده گردید. محصولات جهت تایید، تعیین توالی شدند. آنتی بیوگرام به روش دیسک دیفیوژن برای نمونه‌ها انجام شد و برای تعیین MIC وانکومایسین، روش میکرودایلوشن انجام گردید...

یافته ها: از 400 نمونه جمع آوری شده 278 نمونه انتروکوک تشخیص داده شد. بر اساس روش های فنوتیپی، 86/70% سویه هاانتروکوکوس فکالیس و 46/15% انتروکوکوس فاسیوم و 66/13% سایر گونه‌های انتروکوک بود. با استفاده از روش PCR، 3/72% انتروکوکوس فکالیس و 43/10% انتروکوکوس فاسیوم و 62/17% سایر گونه های انتروکوک تشخیص داده شد. نتایج انتی بیوگرام بیشترین مقاومت را به جنتامایسین و کمترین مقاومت را به لینزولاید نشان داد. 9 مورد مقاوم به وانکومایسین با MIC ≥ 512 بودند.

نتیجه گیری: آنالیز آماری این مطالعه تفاوت معنی داری میان تشخیص سویه های انتروکوکوس به روش PCR نسبت به روش فنوتیپی نشان داد (p ≥ 0.05)؛ در حالی که در تشخیص گونه انتروکوکوس، تفاوت معنی داری مشاهده نشد. وجود مقاومت به وانکوومانسین (MIC ≥ 512mg.ml) در 9 نمونه شیوع قابل توجه مقاومت را نشان داد.

Background: Nowadays, the epidemiology of enterococcal infections has attracted a lot of attention. This led to significant changes occur in the field. Also identification of Enterococcus is important and can be effective to the control flow microorganism antibiotics resistance. So the aim of this study was Identification of Enterococci faecalis & E. faeacium pathogens via Tehran hospitals clinical samples by phenotypic and genotypic methods and evaluation of antimicrobial susceptibility pattern in 2016.

Methods: This study was performed on 400 clinical samples from different hospitals in Tehran, Iran in 2015-2016.Specific cultures were used for identification of Enterococcus. Biochemical test was used to recognize E.faecalis and E.faeacium specious. Then, PCR method was used to identify Enterococcus spp. Amplified DNA sequenced to assure reproduction. Antibiotic susceptibility test was done by disc diffusion method. 

Results: By phenotypic method, 278 Enterococcus were recognized from 400 samples, in which 70.86% were Enterococcus faecalis strains, 15.46% E. faeacium strains and 13.66% were other strains of Enterococcus species. While by using genotypic methods based on PCR, 72.3% were recognized E. faecalis strains, 10.43% E.faeacium strains and 17.62% were strains of other Enterococcus species. Results of antibiograms pattern showed gentamicin was most resistant and least resistant was linezolid. 9 cases were resistant to vancomycin MIC ≥ 512 µg/ml.

Conclusion: Rapid detection can be useful to prevent a massive outbreak of Enterococcus. Also according to the prevalence of vancomycin resistant enterococcus which was shown, the necessity of preventive measures reported. Due to resistance patterns, in order to prescribe the right antibiotics, anti-susceptibility testing for each patient before treatment is recommended.
