بررسی نتایج حاصل از پیوند استخوان مصنوعی در درمان بیماریهای استخوان

چکیده
جهت ترمیم معایب ونقایص استخوانی از پیوند استخوان استفاده می شود. از پیوندهای استخوانی می توان به اتو گرافت( استفاده از نسج استخوان قسمتی از بیمار جهت قسمتی دیگر)‌- آلوگرافت ( استفاده از نسج استخوان از فردی دیگر ) و پیوند استخوان مصنوعی اشاره کرد. پیوند مطلوب و ایده آل اتوگرافت می باشد که به علت عوارض و محدودیت در میزان استخوان و همچنین به علت عوارض و مشکلات تهیه آلوگرافت استفاده از ترکیبات مصنوعی استخوان تحول در ترمیم نقص های استخوانی ایجاد شده است.

استخوان مصنوعی یکی از انواع این ترکیبات می باشد که در مقایسه با سایر ترکیبات مصنوعی کاربرد ساده تر دارد و در کشور ما راحتتر از بقیه ترکیبات در دسترس می باشد.

در محدوده ی زمانی یک سال وطی یک مطالعه توصیفی –تحلیلی ، بیماران دچار بیماریهای استخوانی که اندیکاسیون مصرف پیوند استخوان را داشتند و داوطلب استفاده از پیوند استخوان مصنوعی بودند، انتخاب شدند. پس از بررسی 62نفر (50نفر مرد و12نفر زن) مورد مطالعه قرار گرفتند ، فرمهای مربوطه تکمیل شد وبیماران تحت عمل پیوند استخوان مصنوعی قرار گرفتند و در هفته های چهارم، دهم وبیستم پس از عمل جراحی از نظر نتایج درمانی و جوش خوردگی بررسی شدند.

از 62 نفر مورد مطالعه در 6 نفر  از بیماران پس از هفته بیستم کال استخوانی تشکیل نگردید. بدین ترتیب در7/9 درصد درمان ازطریق استخوان مصنوعی با عدم موفقیت روبرو شد . میانگین سنی بیماران 5/3 ± 33 سال بود که پس از تشکیل گروههای سنی تفاوت معنی داری از نظر زمان تشکیل کال استخوان وجود داشت . زمان تشکیل کال استخوان در گروههای سنی پایین تر در مقایسه با گروههای سنی بالاتر زودتر بود. در هیچ یک از بیماران عوارض عفونی و غیر عفونی مشاهده نشد.

در این مطالعه علاوه بر استفاده از استخوان مصنوعی به تنهایی در بعضی از بیماران خاص مثل عدم جوش خوردن های مشکل استخوان وکیست های استخوانی ، از استخوان مصنوعی همراه با آلوگرافت استفاده شد با توجه به تشکیل کال استخوانی و نتایج مطلوب در 3/90 درصد از بیماران ،مصرف این ماده به تنهایی ویا در موارد خاص به صورت ترکیب با اتوگرافت ویا آلوگرافت در بیماریهای استخوان پیشنهاد می شود.

چکیده English

Introduction:

Bone grafts (Alograft. Autograft and synthetic bonegraft) are used to rapaire bone defects.

According to several technical and medical problems . synthetic bone grafts (B.G) are perefered  to others .

Nova Bone is an ABG which in more available in Iran, now .So we conducted this study to evaluate it.


Methods:

62 Patients with bone defects who needs bone graft during 1 year , under went Nova Bone graft. F/U data we analysed ,according to graft failure and calcium formation.


Results :

6 patients out of 62 had been failed to form calcium after 20 weeks of F/U .(9.7%)

There were no other Complications (infection and noninfectious)


Discussion & Conclusion:

Using SBG (including Nova Bone )has been advocated recently due to several problems in cluding Allografts and Autografts.

Favorable results (90.3% calcium formation )in patients with Nova Bone grafts (alone or in combination with auto & Allograft)Propose the regular use of it for patients need bone grafts.
