چکیده
زمینه و هدف: یکی از مهمترین شاخصهای ارزیابی وضعیت بهداشتی جامعه، وزن هنگام تولد میباشد که به نظر میرسد اعمال خشونت در دوران حاملگی، برآن تاثیر دارد. هدف این مطالعه تعیین ارتباط خشونتهای دوران بارداری با کموزنی هنگام تولد میباشد.
مواد و روشها : در این مطالعهی موردـشاهدی تعداد 318 مادر به عنوان گروه مورد با وزن تولد نوزادان کمتر از 2500 گرم و 318 مادر به عنوان گروه شاهد با وزن تولد نوزادان بیشتر از 2500 گرم که از نظر جنس نوزاد و سن حاملگی همسان شده بودند مورد بررسی قرار گرفتند. گروه مورد به روش در دسترس تا تکمیل نمونه و گروه شاهد به روش سهمیهای با توجه به نسبت تقریبی زایمانها انتخاب شدند. دادهها از طریق پرسشنامه و مصاحبه جمعآوری و به کمک نرم افزارSPSS.13 و با استفاده از آمار توصیفی ارائه و از طریق آزمونهای تی مستقل، مربع کای و رگرسیون لوجستیک چندگانه تحلیل شدند.
یافتهها : میانگین وزن نوزادان مادران گروه شاهد 392±3390 و مورد324±2217 گرم بود. این مطالعه نشان داد در کل 7/13 درصد از خانمها در آخرین بارداری با هیچگونه خشونتی مواجه نشده بودند که این میزان در گروه مورد و شاهد به ترتیب10% و 17% بود. در تحلیل چندگانه از طریق رگرسیون لوجستیک، شانس کموزنی هنگام تولد در مادران دارای مواجهه با خشونت روانی-کلامی و جنسی، به ترتیب 5/1 و 2 برابر مادران بدون مواجهه بود؛ هرچند از لحاظ آماری معنیدار نبود(05/0p>). شانس تولد نوزاد کم وزن مرتبط با خشونت فیزیکی دوران بارداری 9/4 برابر مادران بدون مواجهه بود که این نسبت از نظر آماری معنیدار شد (01/0p=).
بحث و نتیجهگیری : اعمال خشونت در دوران حاملگی، بر وزن هنگام تولد نوزادان تاثیر دارد. بنابراین برنامهریزی گسترده برای کاهش خشونتها، به ویژه خشونت فیزیکی و توجیه خانوادهها در مورد تبعات بعدی آن اهمیت بسزایی دارد.

Assessment of Relationship between Violence Exposure Pregnancy and Low Birth Weight: a caseâcontrol study

Introduction: Birth weight is one of the most important indices of health status of societies. Exposure with violence during pregnancy may influences the birth weight, the aim of this study is assessment of relationship between exposure with violence during pregnancy and low birth weight.
Materials and methods: This case-control study carried out on 636 subjects (318 cases and 318 controls) that were matched by sex of neonate and pregnancy age. The cases selected using convenience sampling method and controls selected using quota sampling method in 2006-2007. The data collected using a structured questionnaire and interview method. The data were analysis by descriptive and analytical (chi-square and independent t test، odds ratio and logistic multiple regression model) statistic، using SPSS 13 software.
Results: The mean of birth weight among cases and controls was 2.217 ± 0.334 and 3.39 ± 0.392 Kg, respectively. This study showed that 13% of women were not exposed with any violence during the pregnancy (10% of cases and 17% of controls). Multiple analysis using logistic regression showed that the chance of LBW among neonates that their mothers exposed with behavioral, sexual and physical violence were 1.5 (P>0.05), 2 (P>0.05) and 4.9 (P=0.01) time more than unexposed group respectively.
Discussion and Conclusion: The results showed that physical violence during pregnancy influence the birth weight. Planning regarding decrease of violence (especially physical one) during pregnancy is highly recommended and justifying the families about its consequences is so important.
