چکیده
مقدمه: ندول تیروئید یک مشکل بالینی شایع و اولین تظاهر بدخیمی می باشد. آسپیراسیون سوزنی تیروئید (fine needle aspiration - FNA) جهت رد بدخیمی در ندول ها به کار میرود. هدف این مطالعه ارزیابی های آسپیراسیون سوزنی تیروئید انجام شده در بیماران مبتلا به ندول تیروئید بین سالهای 1374 تا 1385 از نظر تطابق یافته های سیتولوژی و پاتولوژی بود.
روش کار: در این مطالعه پرونده 1150 بیمار مبتلا به ندول تیروئید بررسی شد که در نهایت 218 بیمار که به دنبال آسپیراسیون سوزنی تیروئید تحت عمل جراحی قرار گرفته بودند و یافته های سیتولوژی و پاتولوژی آنها تواماً موجود بود وارد مطالعه شدند.
یافته ها: از مجموع 218 مورد آسپیراسیون سوزنی پاسخ سیتولوژی در 8/57 درصد خوش خیم، 7/19 درصد بدخیم، 2/20 درصد مشکوک و در 4/1 درصد غیر تشخیصی بود. حساسیت آسپیراسیون سوزنی تیروئید 5/72 درصد، ویژگی آن 83 درصد و دقت 76 درصد محاسبه شد. میزان موارد منفی کاذب در این تست 19 درصد و موارد مثبت کاذب 24 درصد بود.
بحث و نتیجه گیری: حساسیت، ویژگی و دقت آسپیراسیون سوزنی تیروئید در این مطالعه در مقایسه با مطالعات کشورهای دیگر از مقادیر پائین تری برخوردار است. نکته قابل توجه در این مطالعه گزارش موارد غیر تشخیصی کم (4/1 درصد) در مقایسه با سایر مطالعات (17 درصد) و وجود موارد بالای منفی کاذب (19 درصد) در مقایسه با سایر مطالعات (2/5 درصد) می باشد. در مجموع به نظر می رسد که بیوپسی آسپیراسیون با سوزن نازک کماکان یک روش دقیق و مقرون به صرفه در تعیین ماهیت ندول تیروئید است.

Evaluation of thyroid fine needle aspiration for pathological concordance

Introduction:
Thyroid nodules are a common problem and the first thyroid malignancy presentation in clinical practice. The purpose of this study was to evaluate FNABs performed in patients with thyroid nodule between years 1995 to 2006 for cytohistologic concordance.
Material and Methods:
In this study we reviewed files of 1150 patients with thyroid nodule and finally 218 patients undergone thyroid surgery following FNAB biopsy were chosen for the study.
Results:
Four cytologic diagnostic categories were used and the rates of these categories were as follows: benign, 57.8 percent; suspicious for malignancy, 20.2 percent; malignant, 19/7 percent and nondiagnostic, 1.4 percent. We obtained a sensitivity of 72.5 percent, a specificity of 83 percent and an accuracy of 76 percent. The false negative rate was 19 percent and the false positive rate was 24 percent.
Conclusion:
The results of this study showed that diagnostic value (sensitivity, specificity, accuracy) of FNAB in our region in comparison with other countries is low. The most concerning result is the low rate of nondiagnostic (1.4 vs. 17 percent) and high rate of false negative reports (19 percent) in comparison with other studies (5.2 percent). It seems that FNA is an accurate and cost effective procedure yet and it can be more accurate with better sampling and more attention to false negative and inadequate results. Considering of a standard classification and clinical data is suggested especially in suspicions results.
