ارزیابی کارایی و عوارض تزریق فرآورده‌های خونی در بیمارستان‌های جهرم: یک مطالعه مقطعی

چکیده
مقدمه: رزرو خون در جراحی ها بایستی طبق دستورالعمل استاندارد انجام شود تا از هدر رفت خون جلوگیری گردد. با توجه به چالش‌های موجود در زمینه کاربرد مناسب فرآورده‌های خونی و اهمیت پایش عوارض انتقال خون، این مطالعه با هدف ارزیابی کارایی و عوارض تزریق خون در بیمارستان‌های جهرم انجام شد.
مواد و روش ها: در این مطالعه مقطعی، 3400 فرم درخواست خون از دو بیمارستان‌ پیمانیه و شهید مطهری جهرم در سال 1403 بررسی شد. با استفاده از داده های استخراج شده، میزان استفاده از فرآورده های خونی، نسبت کراسمچ به تزریق (C/T) و همچنین عوارض حاد تزریق خون تعیین گردید. آنالیز داده ها با 26-SPSS و آزمون های تی تست و آنوا انجام شد.
یافته ها: از ۳400 فرم درخواست خون، برای 1951 بیمار (3/57 درصد) کراسمچ انجام شد و 728 بیمار (1/31 درصد) فراورده خونی (1226 واحد) دریافت کردند. شایعترین فراورده درخواستی پک سل (9/58 درصد) و شاخص کلی C/T برابر 16/4، شاخص تزریق (TI) برابر ۰٫6۳ و احتمال تزریق (T%) برابر 4/41 درصد بود. بالاترین شاخص C/T در بخش‌های انکولوژی (2/7) و زنان و زایمان (1/6) مشاهده شد. عوارض حاد مرتبط با تزریق فراورده در 28 بیمار (9/3 درصد) رخ داد.
نتیجه گیری: مطالعه حاضر نشان‌دهنده درخواست‌های غیرضروری خون و بروز عوارض تزریق به ویژه در بخش های انکولوژی و زنان و زایمان است. این یافته‌ها بر ضرورت بازنگری در پروتکل‌های موجود، تاکید بر اجرای بومی جدول حداکثر سفارش خون برای جراحی (MSBOS) و آموزش هدفمند پرسنل جهت ارتقای کیفیت خدمات انتقال خون تأکید دارد.

Evaluation of the efficacy and complications of blood product transfusion in Jahrom hospitals: A Cross-Sectional Study

Background: Blood reservation in surgical operations should be based on a standard protocol. Considering the existing challenges in the appropriate use of blood products and the importance of monitoring blood transfusion complications, this study was conducted to evaluate the efficacy and complications associated with blood product transfusions in Jahrom hospitals.
Materials and Methods: In this cross-sectional study, 3,400 blood request forms from Paymanieh and Shahid Motahari hospitals in Jahrom were reviewed during 2024. Using the extracted data, the amount of blood products used, the crossmatch-to-transfusion ratio (C/T), and the complications related to blood transfusion were determined. The data were analyzed using SPSS-26, t-test, and ANOVA.
Results: Out of 3400 blood requests, crossmatch was performed for 1951 patients (57.3 %), and 728 patients (31.1 %) received products (1228 units). The percentage of requested packed cell products was 58.9%, and the overall C/T ratio was 4.16, a transfusion index (TI) of 0.63, and a transfusion probability (T%) of 41.4%. The highest C/T ratios were observed in oncology (7.2) and obstetrics-gynecology (6.1) departments. Acute complications related to product transfusion occurred in 28 patients (3.9%). A significant association was observed between Male physicians and complications (P<0.004).
Conclusions: The present study indicates unnecessary transfusion demands and the occurrence of adverse complications, specifically in oncology and obstetrics-gynecology departments. These findings emphasize the need for a review of existing protocols, an emphasis on implementing the local MSBOS program, and continuous training of personnel to enhance the quality of blood transfusion services.
