آیا کلونیدین خوراکی اثری بر کنترل درد بعد از عمل جراحی کوله سیستکتومی به روش لاپاراسکوپیک دارد؟ یک مطالعه کارآزمایی بالینی تصادفی شده دو سویه کور

چکیده
مقدمه: درد بعد از اعمال جراحی یک تجربه ی ناخوشایند است. این فرایند باعث می شود اختلالات فیزیولوژیک در تمام سیستم های بدن بوجود آید. کلونیدین به عنوان یک داروی الفا دو آگونیست یکی از داروهای مورد استفاده جهت کنترل درد بعد از عمل جراحی
کله سیستکتومی لاپاراسکوپی می باشد. لذا این مطالعه با هدف بررسی تاثیر کلونیدین خوراکی بر کنترل درد و میزان مصرف مخدرها بعد از عمل جراحی کله سیستکتومی لاپاراسکوپی انجام شد.
روش کار: در این مطالعه کارآزمایی بالینی تصادفی شده دو سویه کور 50 بیمار با کلاس بیهوشی I و  II تحت عمل جراحی
کله سیستکتومی لاپاراسکوپی قرار گرفتند. بیماران به صورت تصادفی به دو گروه کلونیدین خوراکی و کنترل تقسیم شدند. فشار خون سیستول و دیاستول، فشار متوسط شریانی و ضربان قلب حین عمل ثبت شد. میزان درد در ریکاوری، 6، 12 و 24 ساعت پس از عمل انداره گیری شد.  میزان مصرف مخدر بعد از عمل در هر دو گروه ثبت گردید. تجزیه و تحلیل اطلاعات با استفاده از نرم افزار SPSS نسخه 21 و با استفاده از آزمون های آماری توصیفی و استنباطی صورت گرفت.
یافته ها: گروه های موردمطالعه از نظر متغیرهای دموگرافیک سن، وزن، قد و شاخص توده بدنی و جنسیت همسان هستند. نتایج آزمون کای دو نشان داد که بین گروه های کلونیدین وکنترل از لحاظ درد در زمان های صفر، 6ساعت، 12ساعت و 24 ساعت تفاوت معنی دار وجود داشت ( 0.001>P). نتایج آزمون کای دو نشان داد که تفاوت معناداری بین دو گروه کنترل و کلونیدین در دریافت داروی پتدین وجود داشت (p=0.021).
نتیجه گیری: نتایج مطالعه حاضر نشان داد که کلونیدین خوراکی اثر بخشی بهتری در کنترل شدت درد و میزان مصرف مخدر نسبت به گروه کنترل دارد. بنظر می رسد در اعمال جراحی کله سیستکتومی لاپاراسکوپی کلونیدین خوراکی اثربخشی خوبی در کنترل درد و میزان مصرف مخدر پس از عمل داشته باشد.

Introduction: Postoperative pain is an unpleasant experience. This process causes physiological disorders in all systems of the body. Clonidine as an alpha dual agonist is one of the drugs used to control pain after laparoscopic cholecystectomy. Therefore, this study was performed to evaluate the effect of oral clonidine on pain control and opioid use after laparoscopic cholecystectomy.
Materials and Methods: In this randomized double-blind clinical trial study, 50 patients with class I and II anesthesia underwent laparoscopic cholecystectomy. Patients were randomly divided into oral and control clonidine groups.
Systolic and diastolic blood pressure, mean arterial pressure and intraoperative heart rate were recorded. Pain was measured in recovery, 6, 12 and 24 hours after surgery. Postoperative drug use was recorded in both groups. Data analysis was performed using SPSS software version 21 and using descriptive and inferential statistical tests.
Results: The studied groups are similar in terms of demographic variables of age, weight, height, body mass index and gender. The results of chi-square test showed that there was a significant difference between clonidine and control groups in terms of pain at zero, 6 hours, 12 hours and 24 hours. (P<0.001). The results of chi-square test showed that there was a significant difference between the control and clonidine groups in receiving pethidine. (P= 0.021).
Conclusion: The results of the present study showed that oral clonidine is more effective in controlling pain intensity
and drug use than the control group. Oral clonidine laparoscopic cholecystectomy surgery seems to be effective in controlling pain and postoperative drug use.
