چکیده
مقدمه: افزایش مشخص فشارخون و ضربان قلب بیشتر اوقات در خارج کردن لوله تراشه اتفاق می افتد. این اثرات، هشدار دهنده ولی گذرا هستند. مطالعه حاضر با هدف مقایسه کتامین و دکسمدتومیدین در حفظ ثبات همودینامیک در جراحی کله سیستکتومی لاپاراسکوپی با بیهوشی عمومی انجام شد.
روش کار: این مطالعه یک کارآزمایی بالینی تصادفی شده دو سویه کور بود. تعداد 90 بیمار 18 تا 44 سال (در سه گروه 30 نفری) حین بیهوشی عمومی مورد ارزیابی قرار گرفتند. معیار های ورود به مطالعه شامل قرارگیری در کلاس بی هوشی (ASA) یک یا دو و رضایت به شرکت در مطالعه بود. بیماران به طور تصادفی به سه گروه کنترل، گروه مداخله با کتامین و گروه مداخله با دکسمدتومیدین تقسیم شدند. علایم همودینامیک بیماران شامل فشار خون سیستول، فشار خون دیاستول، فشار متوسط شریانی، درصد اشباع اکسیژن خون و ضربان قلب اندازهگیری شدند. تجزیه و تحلیل دادهها با شاخصهای آمار توصیفی همچون میانگین، درصد فراوانی و انحراف معیار و آزمونهای آمار استنباطی همچون من ویتنی و کروسکال والیس به کمک نرم افزار SPSS نسخه 21 انجام شد.
یافتهها : یافتههای پژوهش نشان داد میانگین فشارخون سیستول، فشار خون دیاستول و متوسط شریانی تفاوت معنادار آماری بین سه گروه کتامین، دکسمدتومیدین و کنترل در زمانهای قبل از القا، بعد از القا، 5 دقیقه بعد، 15 دقیقه بعد، 30 دقیقه بعد، 60 دقیقه بعد، در ریکاوری و خارج از ریکاوری وجود دارد (0.05> P)، اما ضربان قلب بین سه گروه در این مقاطع زمانی تفاوت معناداری نداشتند (0.05< P). همچنین میانگین درصد اشباع اکسیژن خون در گروه کتامین پایینتر از گروههای دکسمدتومیدین و کنترل بود (0.05> P).
نتیجه گیری: براساس نتایج مطالعه حاضر در جراحی کله سیستکتومی لاپاراسکوپی با بیهوشی عمومی، استفاده از دکسمدتومیدین در مقایسه با کتامین باعث ثبات همودینامیک میشود.

Introduction: One of the basic features of a successful anesthesia is reversible loss of consciousness with immobility, lack of awareness, not responding to painful stimulation and not remembering surgical interventions. The present study was conducted with the aim of comparing ketamine and dexmedetomidine in maintaining hemodynamic stability in laparoscopic head cystectomy surgery with general anesthesia.
Materials and Methods: This study is a double-blind randomized clinical trial Study. 90 patients aged 18 to 44 (in three groups of 30) were evaluated during general anesthesia. Inclusion criteria included anesthesia class (ASA I, II) and consent to participate in the study. Patients were randomly divided into three groups: 1) control and 2) intervention with ketamine, intervention and 3) intervention with dexmedetomidine. Patients' hemodynamic symptoms (systolic blood pressure, diastolic blood pressure, mean arterial pressure, blood oxygen saturation percentage and heart rate) were evaluated. Data analysis was done using descriptive statistics (mean, percentage and standard deviation) and inferential statistical tests (Mann-Whitney and Kruskal-Wallis) using SPSS version 21 software.
Results: The findings of the research showed that between the three groups of ketamine, dexmedetomidine and control in the times before induction, after induction, 5 minutes after, 15 minutes after, 30 minutes after, 60 minutes after, in recovery and outside of recovery, from There was a significant difference in mean systolic and diastolic blood pressure and mean arterial blood pressure (P<0.05). But there was no significant difference in heart rate between the three groups at these times (P<0.05). The average percentage of blood oxygen saturation in the ketamine group was lower than the dexmedetomidine and control groups (P<0.05).
Conclusion: Based on the results of the present study, the use of dexmedetomidine in laparoscopic cholecystectomy surgery compared to ketamine causes hemodynamic stability in laparoscopic cholecystectomy surgery with general anesthesia.
