چکیده
مقدمه: عفونت های باکتریایی مجاری ادراری یکی از شایع ترین عفونت ها در دوران بارداری محسوب می شوند. اکثر باکتریهای مزبور مولد بیوفیلم و مقاوم به ترکیبات ضد میکروبی هستند. بنابراین، هدف از مطالعه حاضر تعیین الگوی مقاومت آنتی بیوتیکی و میزان تشکیل بیوفیلم توسط ایزوله های اشریشیا کلی و کلبسیلا پنومونیه حاصل از نمونه های ادراری زنان باردار در کرمان بود.
روش کار: نمونه ادرار 107 زن باردار بررسی شد. ایزوله ها با آزمون های بیوشیمیایی شناسایی و برای تعیین حساسیت نسبت به
آنتی بیوتیک ها به روش انتشار دیسک و حداقل غلظت ممانعت کننده از رشد ارزیابی شدند. هیدروفوبیسیتی سطح سلول و توانایی تشکیل بیوفیلم بر سطوح شیشه و پروپیلن در شرایط سکون و حرکت تعیین شدند. قابلیت اثرآنتی بیوتیک ها بر میزان تشکیل بیوفیلم بر سطح کاتتر ادراری مشخص شد.
یافته ها: از 80 نمونه مثبت، 35 ایزوله اشریشیا کلی و 15 ایزوله کلبسیلا پنومونیها انتخاب شدند. بیشترین مقاومت و حساسیت به ترتیب نسبت به آموکسی سیلین و ایمی پنم تعیین شد. موارد با هیدروفوبیسیتی بیشتر از 70% قابلیت بهتری در تشکیل بیوفیلم به خصوص بر سطح پروپیلن در حالت متحرک نشان دادند. کشت با رقت های 0.25×MIC و 0.5 از آنتی بیوتیک های انتخابی، توانایی چسبندگی ایزوله ها بر سطح کاتترهای ادراری را کاهش داد.
نتیجه گیری: باید پژوهش های مشابه به طور مداوم در مراکز پزشکی مختلف در راستای ارزیابی حساسیت ضدمیکروبی و تعیین میزان بیوفیلم صورت پذیرد تا بر اساس نتایج، راه کارهای مناسب تر درمانی برای جلوگیری از تشکیل بیوفیلم انتخاب و بهترین عملکرد درمانی در ارتباط با مبتلایان به عفونتهای ادراری افراد باردار ارایه شود.

Biofilm formation and antimicrobial resistance of uropathogenic Escherichia coli and Klebsiella pneumonia strains isolated from pregnant women in Kerman, Iran

Introduction: Urinary tract infections are the common medical problems in pregnant women and associated bacteria
produce biofilm which protects them from antibiotics. Aim of this study was to determine the pattern of antibiotic resistance and biofilm formation by Escherichia coli and Klebsiella pneumoniae isolated from urine of pregnant women in Kerman, Iran.
Materials and Methods: 107 urine samples were collected from pregnant women. Identification was performed by biochemical tests. Antibiotic sensitivity by disc diffusion method and minimum inhibitory concentration (MIC) by
agar dilution method were performed. The cell surface hydrophobicity (CSH) behavior of those strains and their ability to form biofilm on glass and propylene surfaces was evaluated in static and shake conditions. The effect of antibiotics on biofilm formation ability was also determined on the catheter surface.
Results: From 80 positive cases, 35 isolates of Escherichia coli and 15 isolates of Klebsiella pneumoniae were selected for this research. The Maximum resistance and sensitivity of isolates were observed about amoxicillin and imipenem, respectively. All isolates with CSH higher than 70% showed better biofilm formation even though on propylene in shaking state. Treatment of culture with 0.5 MIC (μg/ml) and 0.25 MIC (μg/ml) of selected antibiotics significantly reduced the adhesion ability of isolates.
Conclusion: The survey therefore came to the conclusion that similar researches should be continuously repeated in
different medical centers to test antimicrobial susceptibility as well as diagnosing and quantifying biofilm infection which surely achieved data help to find appropriate therapeutic way against UTI in pregnancy.
